The cooldown for shooting your friends needs to be a lot shorter... I have reflexes that kill my friends lol...
But I like the classic no upgrades, it makes it interesting.

The organs need to restore more hp, because whenever I fought a boss it took down all that HP I regained.

Maybe you could make Madness Interactive 2, with more details & a bigger screen.
Maybe switch some guns for others possible.
Like instead of the urinal, maybe a chainsaw or pipe?
